Appointment and Office Hours
Our office hours are Monday through Friday from 7:45 a.m. until 5:00 p.m. To schedule an appointment, please call {tel} during regular office hours. Please bring insurance cards, eyeglasses, contact lenses, and an updated list of medications to each examination.
New Patients
We welcome new patients to our practice. In order to expedite your office visit, please download the New Patient form, Acknowlegement Of Receipt Of our Notice of Privacy Practices form and complete both forms prior to coming in for your examination. Remember to bring these forms, your eyeglasses, contact lenses, insurance cards, and medication list with you.

Billing and Insurance
We participate with most insurance companies, PPOs and HMOs. Please familiarize yourself with your individual plan. For procedures not covered by your insurance and for co-pays, we require payment at the time of your visit. We accept VISA, Mastercard, Discover, personal checks and cash.
Emergencies
Emergencies frequently arise after normal office hours or on weekends or holidays. If necessary, please call our office at {tel} and our answering service will connect you to the on-call ophthalmologist. There is an ophthalmologist available 24 hours a day, 7 days a week.
